Clinical Liaison Home Health

Responsibilities

• Affords patients with guidance through post-acute care processes.
• Utilizes evidence-based clinical guidelines, preventative strategies, protocols, and metrics in developing transition plans tailored to individual patient needs.
• Engages in strategic partnerships with health systems, hospitals, inpatient facilities, physicians, and physician groups.
• Participates in executive-level discussions and negotiations.

Qualifications

• Graduate of an approved school of nursing, therapy, or social work.
• Licensed in the state where practicing.
• Two (2) years of demonstrated field experience.
• Demonstrated understanding of federal, state, and local laws and regulatory guidelines applicable to Medicare certified home health and hospice operations.
• Basic technology skills, including proficiency in operating a mobile device.

Requirements

• Valid state driver license.
• Automobile liability insurance as required by law.
• Dependable transportation in good working condition.
• Safety in driving an automobile under all weather conditions.
