Responsibilities
- Collaborate with healthcare professionals, residents, and families to coordinate comprehensive care plans that meet the unique needs of each individual
- Develop and maintain relationships with healthcare providers, hospitals, and other external agencies to ensure seamless transitions and continuity of care
- Conduct initial assessments, identify risks, and develop individualized care plans to mitigate potential health issues
- Educate residents, families, and staff on healthcare procedures, medication management, and wellness strategies
- Maintain accurate and detailed records of patient interactions, assessments, and care plans
- Participate in multidisciplinary care conferences and team meetings to ensure effective communication and collaboration
- Identify and report any concerns or issues related to resident health and well-being to the healthcare team and management
Requirements
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Missouri
- Experience and skills as a clinical/healthcare liaison in a long-term care/skilled nursing or related field
