About the role

Organizes and coordinates the day-to-day activities of the Pharmacy department, ensuring compliance with all federal and state laws. Serves as the liaison between the department and staff, physicians, patients, family, and other hospital staff. Delivers pharmaceutical care through patient-centered clinical service, medication information, education, medication preparation, and distribution to ensure safe, effective, and cost-efficient medication therapy.

Exercises sound judgment and developed clinical skills to provide input and implement the patient's plan of care based on diagnosis in a timely manner. Builds confidence and cooperation through competent patient assessment, attentive monitoring, and effective communication with patients, their families, support groups, and other healthcare providers. Adheres to all local, state, and federal regulations, codes, policies, and procedures to ensure privacy and safety while delivering optimal patient care.

May also be responsible for performing specific tasks and/or orienting other staff to the department. Works as a lead with pharmacy staff to fulfill regulatory, operational, and clinical needs of pharmacy services (no direct reports). Assists the management team in project and initiative implementation, such as IV room lead, clinical lead at small affiliates, or PIC at large affiliates with multi-campus pharmacies.

Responsibilities

• Deliver pharmaceutical care through patient-centered clinical service, medication information, education, and medication preparation and distribution.
• Implement the patient’s plan of care based on diagnosis in a timely manner.
• Assess, monitor, and communicate effectively with patients, families, and healthcare providers.
• Ensure compliance with all federal, state, and local regulations, including privacy and safety standards.
• Lead pharmacy staff to meet regulatory, operational, and clinical needs (no direct reports).
• Assist in project and initiative implementation (e.g., IV room lead, clinical lead, or PIC at multi-campus affiliates).
• Prioritize and delegate assignments while working within standardized policies and procedures.
• Identify, evaluate, and resolve standard problems using established solutions.
• Maintain patient privacy and confidentiality of PHI.
• Collaborate with peers and other healthcare providers to achieve departmental and corporate objectives.

Requirements

• Doctorate: Graduation from an accredited school of Pharmacy.
• Licensure: PHARMD – Licensed Pharmacist or PHARMR – Registered Pharmacist (preferred).
• 3+ years of experience as a Pharmacist in an acute care setting.
• APP licensure and relevant experience in transitions of care role preferred.

Skills and Knowledge

• Comprehensive knowledge of current pharmacological/biopharmaceutical principles, medical terminology, pathologies, disease states, and related information for a medical management plan.
• Basic working knowledge of legal requirements and accreditation standards, including NABP, TJC, Title XXII, DHS, DEA, FDA, and USP.
• Strong written and verbal communication skills to explain sensitive information clearly and professionally to diverse audiences, including non-medical individuals.
• Well-developed time management and organizational skills, with the ability to prioritize assignments and meet deadlines.
• Proficiency in computer applications such as Microsoft Office Suite (Word, Excel, Outlook), related pharmaceutical technology, EHR, and EPIC.
• Ability to work independently and as part of a team, handling multiple tasks in an environment with interruptions.

Schedule

• Full-time (40 hours per week).
• Shift: Varied (10-hour shifts).
• Days of the week: Variable.
• Weekend requirements: Every other weekend.

Pay

Pay range: $91.28 - $127.78 per hour. The salary range may vary above or below the posted range based on location and other factors, including experience, education, skills, licensure, and certifications.
