Clinical Intake Liaison (LPN)

Responsibilities

• Review all referrals in a timely manner.
• Maintain a working knowledge of community’s practices related to payor and hospital contracts.
• Work alongside the insurance liaison for review.
• Communicate with referral sources regarding referral status.
• Communicate with the facility to coordinate admissions.
• Maintain confidentiality of protected health information, including verbal, written, and electronic communications.

Requirements

• Current unrestricted LPN license for the state of Tennessee.
• Experience in a nursing or Admissions position in a health care facility.
• Experience in SNF preferred.
