About the role
North Community Counseling Centers (NCCC) is a nonprofit mental health agency founded in 1968. We offer a range of services including substance disorder treatment, youth and adolescent programming, and medication management. Our Assessment Specialist position is open for individuals with LSW, LPC, or MFT licenses.
Responsibilities
- Works closely with hospital staff including discharge planners, physicians, social workers, case managers, inpatient units and administration to provide direct and indirect care to clients.
- Works with hospital social worker to begin the housing process if needed.
- Develops and maintains new and current relationships with assigned hospital(s).
- Provides initial and ongoing diagnostic assessments of both hospital and agency clients in the hospital and for the agency and completes the Hospital ISP.
- Provides BH counseling and CPST for clients through home, office and hospital visits.
- Continues to work with clients for up to three months upon discharge to ensure proper and complete linkage with resources and continuity of care.
- Attends treatment team meetings.
Qualifications
- Must have an LSW/LPC/MFT license or higher.
- Previous experience in social work or counseling.
- Strong documentation skills and ability to use electronic health record system.
- Careful and caring demeanor.
- Strong leadership qualities.
- Excellent written and verbal communication skills.
- Able to pass a background check and drug test.
