The Clinical Transition Specialist is an experienced sales professional who uses sales techniques to sell Option Care products and services to discharging patients. They educate patients, families, and facility staff about how services and products will be facilitated at an alternative site, ensuring proper placement within the Home Health Care setting.

This role requires proactive relationship-building with referral sources and collaboration with Account Managers to grow referral rates and achieve sales goals. Responsibilities

  • Proactively maintain and grow relationships with referral sources to increase sales and patient starts.
  • Serve as the key point of contact and representative of Option Care, providing education, assistance, and service to referral sources.
  • Interact with area service providers daily to sell Option Care services for discharging patients.
  • Conduct assessments of patients selected by hospitals to ensure viability for alternative site care.
  • Use sales techniques to educate patients, families, and facility staff about Option Care services and products.
  • Provide hands-on, in-person education to patients and caregivers to facilitate timely discharge and therapy independence.
  • Communicate effectively with agency staff, medical teams, patients, and families throughout the discharge process to implement an effective treatment plan.
  • Collect, review, and complete pre-admission information, securing necessary sign-offs.
  • Partner with Account Managers and Regional Sales Directors to create and execute area business and growth plans.
  • Collaborate with Account Managers and marketing staff to deliver educational and promotional programs to patients, caregivers, and referral sources.
  • Review patients’ medical records to obtain pertinent medical history and insurance information, communicating details to the patient registration department.
  • Initiate care transition coordination with referral sources and internal partners to ensure seamless patient transitions to home or alternate treatment sites (ATS).
  • Participate in data collection for therapy starts and patient tracking, including facilitating order transfers via phone, fax, and e-prescribing.
  • Maintain confidentiality of patient and proprietary information, adhering to legal guidelines for safeguarding sensitive data.
  • Provide oversight and input to providers regarding patient assessment, treatment processes, and transition to home care.
  • Serve as a point of contact, coordination, and communication with other providers.
  • Arrange for any special medical supplies or appliances needed for patient care.

Requirements

  • Licensed Registered Nurse (RN) or licensed Pharmacist in the state of practice.
  • At least 2 years of experience in the healthcare industry.
  • Experience establishing and maintaining relationships with individuals at all levels of an organization, the business community, and vendors.
  • Knowledge of standard practices for services offered and current relevant standards (e.g., ACHC, URAC).
  • Experience providing customer service to internal and external customers, including meeting quality standards and evaluating customer satisfaction.
  • Basic PC skills: proficiency in internet, email, Microsoft Word, Excel, and PowerPoint.
  • Experience identifying operational issues and recommending strategies to resolve and improve processes.
  • Access to reliable transportation for travel to care facilities, home visits, and multiple hospitals; valid driver’s license and proof of insurance required if using a personal vehicle.
  • Ability to plan, organize, and deliver presentations.

Preferred Qualifications

  • Hands-on experience in home or alternate site infusion or discharge planning.
  • Experience growing service provider partnerships.

Pay

Salary to be determined by the applicant's education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data. Pay range: $88,853.42–$148,111.54.

Schedule

Travel is required to and from partnership facilities, community hospitals, and medical practice offices to sell Option Care Services, process referrals, and provide live education and training support to patients, caregivers, and referral sources.
