Clinical Evaluator

What You'll Do

• Conduct thorough assessments of residents' health conditions, functional abilities, and care requirements.
• Drive the development of personalized care plans, ensuring the highest standard of care and well-being of our residents.
• Perform comprehensive assessments of physical, mental, and emotional health to determine appropriate care setting.
• Evaluate functional abilities and care needs through detailed observations and assessments.
• Collaborate with interdisciplinary teams to develop individualized care plans.
• Establish and maintain positive relationships with area hospitals and community agencies.
• Develop and maintain relationships with social workers, case managers and other contacts in the healthcare and payor community.
• Educate residents and families about facility services, procedures, and care expectations.
• Monitor and reassess residents' progress and update care plans accordingly.
• Advocate for residents' well-being and coordinate with healthcare providers to address needs.
• Document assessments and care plans accurately and timely.

Qualifications of a Clinical Evaluator

• 3 years of clinical or case management experience in long-term care
• Experience in discharge planning and marketing activity
• A compassionate and empathetic approach to patient care
• An interest in the nursing needs of the aged and the chronically ill with the ability to work with both
• Strong clinical skills and critical thinking abilities
• Excellent communication and interpersonal skills
• The ability to work effectively in a multidisciplinary team environment

Pay

Rate of Pay: USD $38.00 - USD $40.86 /Hr.
