What You'll Do
- Conduct thorough assessments of residents' health conditions, functional abilities, and care requirements.
- Drive the development of personalized care plans, ensuring the highest standard of care and well-being of our residents.
- Perform comprehensive assessments of physical, mental, and emotional health to determine appropriate care setting.
- Evaluate functional abilities and care needs through detailed observations and assessments.
- Collaborate with interdisciplinary teams to develop individualized care plans.
- Establish and maintain positive relationships with area hospitals and community agencies.
- Develop and maintain relationships with social workers, case managers and other contacts in the healthcare and payor community.
- Educate residents and families about facility services, procedures, and care expectations.
- Monitor and reassess residents' progress and update care plans accordingly.
- Advocate for residents' well-being and coordinate with healthcare providers to address needs.
- Document assessments and care plans accurately and timely.

Qualifications of a Clinical Evaluator
- 3 years of clinical or case management experience in long-term care
- Experience in discharge planning and marketing activity
- A compassionate and empathetic approach to patient care
- An interest in the nursing needs of the aged and the chronically ill with the ability to work with both
- Strong clinical skills and critical thinking abilities
- Excellent communication and interpersonal skills
- The ability to work effectively in a multidisciplinary team environment
Pay
Rate of Pay: USD $38.00 - USD $40.86 /Hr.
