Responsibilities
- Apply the full Nutrition Care Process and medical nutrition therapy for geriatric residents using strong clinical judgment, counseling skills, cultural competence, critical thinking, and effective written and verbal communication.
- Conduct and document thorough nutrition assessments, complete MDS requirements, evaluate medical, psychosocial, functional, and behavioral factors, supervise data collection, and identify nutrition-related risks requiring intervention or referral.
- Analyze assessment data to identify and prioritize nutrition diagnoses, document PES statements, and develop evidence-based, individualized nutrition care plans tailored to resident safety, needs, and preferences.
- Implement nutrition interventions, provide resident and family education and counseling, coordinate care with interdisciplinary teams, delegate appropriately, and adjust plans based on resident response and treatment goals.
- Monitor outcomes through meal observations, progress tracking, and outcome measures; communicate with physicians and care teams; document care accurately; support discharge planning; and serve as a nutrition resource to staff, residents, and families.
- Ensure compliance with HIPAA, CMS, state regulations, and accreditation standards; promote a positive, inclusive workplace; assist with surveys, audits, student education, and operational support to enhance quality and safety.
Qualifications
- Must have a bachelor’s degree in dietetics, foods and nutrition, or related area, granted by a U.S. regionally accredited college or university.
- Completion of a didactic program in dietetics and supervised practice program approved by the Commission on Accreditation for Dietetics Education (CADE) of the American Dietetic Association.
- Must be registration eligible and licensed within 1 year of employment.
- Experience in healthcare, skilled nursing facility or assisted living community 1-2 years preferred.
