About the Role
Provides nutrition care to Florida PACE Centers participants in all care settings.
Responsibilities
- Responsible for providing nutrition education to participants and caregivers as ordered by physician or requested by nursing or other disciplines
- Responsible for performing nutritional assessments and periodic reassessments as indicated by need and FPC standards
- Participates in care planning for initiation, progression, and maintenance of participants receiving tube feedings, TPN and PPN
- Attends multi-disciplinary team conferences for FPC participants
- Provides nutrition education to staff and community as indicated
- Works collaboratively with other care disciplines in development of the team plan of care
Requirements
- BS or advanced degree from an accredited college with major studies in food and nutrition or dietetics
- One year experience working with the frail elderly population
- Licenses/Certifications: Current Florida Dietitian/Nutritionist license, ADA registered or Registered Dietitian eligible

- Requires ability to teach, analytical thinking and ability to provide care to geriatric participants
