About the Role
Per diem position with hours scheduled based on department needs at Elmhurst and Edward Hospital locations in Elmhurst, IL and Naperville, IL. Schedule includes Monday-Friday, 8am - 4:30pm with Saturday coverage every 6 weeks.
Responsibilities
- Designs and develops consumer nutrition, health, and wellness education programs and/or services targeting current user groups and data collection of risk indicators.
- Identifies and develops evaluation criteria for assessing effectiveness of these programs.
- Performs nutritional, health, and wellness screening and assessment.
- Manages ongoing nutritional care plans based upon the developmental needs of each patient-member.
- Prepares and completes patient-member records and documentation support according to licensure guidelines.
- Contributes to the nutrition-specific education/in-service training of patients, professional staff, colleagues, support staff, and students when required.
- Provides appropriate documentation that summarizes the nutrition care plan in the patient’s medical record, including nutritional assessment, diagnosis plan, implementation, and progress toward needs.
- Demonstrates accountability for the proper use of patient’s protected health information.
- Utilizes patient scheduling and monitors/maintains patient records through electronic medical records.
Requirements
- Master’s Degree in Nutrition, required.
- Completion of required nutrition and dietetic coursework through ACEND-Accredited didactic program or coordinated program in dietetics.
- Completion of an ACEND-accredited supervised practice through a dietetic internship, individualized supervised practice pathway, or a coordinated program in nutrition and dietetics.
- Current State of Illinois Dietitian Nutritionist licensure (LDN) within 90 days of start date.
- Registered Dietitian Nutritionist credential (RDN).
- Minimum of one year of clinical dietitian experience, preferred.
