About the role
The Clinical Diabetes Educator at NCH works with a group of high-risk, high-cost, complex patients to provide education on diabetes management and coordinate their care. This role involves serving as a patient advocate, educating patients and families, collaborating with healthcare teams, and participating in research and community programs.
Responsibilities
- Provides education on diabetes self-management and survival skills to promote patient compliance with medical regimen.
- Assesses patient needs and collaborates with nursing and social services staff to develop discharge plans.
- Maintains flexible work patterns to meet the needs of staff and patients.
- Communicates with health team members and ancillary departments to ensure efficient and accurate communication.
- Develops and provides diabetes education programs and equipment training/testing to nursing and support staff.
- Facilitates educational/behavioral experiences to assist patients in achieving identified goals.
- Participates in focused studies and protocol development to enhance outcomes.
- Promotes NCHHCS by supporting diabetes education/awareness programs in the community.
- Engages in professional development and continuing education to maintain current knowledge and skills.
- Affirms the fiscal efficiency of diabetes education and nutrition programs.
- Assesses and reviews own performance and participates in peer reviews.
Requirements
- Licensed as a Registered Nurse in the state of Florida and a minimum of one year of experience in diabetes care, or certified as a Certified Diabetes Educator (CDE) with two years' experience as a Dietitian.
- Basic Life Support (BLS) certification from the American Red Cross or American Heart Association.
- Bachelor of Science in Nursing preferred.
- Intermediate computer knowledge: proficiency in using Microsoft Word, Excel, Outlook, and Windows.
