Clinical Coordinator-START Program

About the role

Join our team as a Full Time START CSIDD Coordinator in Rochester. Work from home flexibility allows you to make a positive impact from the comfort of your own space. Earn between $60,405.80 - $65,405.80 with a $3,000 sign on bonus. Additionally, an annual pay increase of $3,000 will be applied upon successful completion of START Certification.

Responsibilities

• Maintain an active caseload of individuals referred for services.
• Develop cross-system plans and provide outreach, consultation, and crisis supports.
• Guide individuals towards the help and resources they require through innovative problem-solving skills and an empathetic approach.
• Manage a caseload effectively with strong organizational abilities.
• Provide clear communication skills for outreach and consultation.
• Work alongside a team passionate about making a difference in the lives of others.
• Support individuals with developmental disabilities who present with complex behavioral and mental health needs.
• Provide community-based person-centered supports enabling individuals to remain in their home or community residence.
• Offer services including Therapeutic Resource Centers, In Home Support, Crisis Response, Consultation, Education, and Training.

Requirements

• A master's degree in social work, Psychology, Counseling, or another Human Services field.
• At least 2 years' experience working with individuals with IDD and mental health/behavioral health needs.
• Experience working with complex systems, social work, mental health, and families is preferred.

Qualifications

• Strong organizational abilities to manage a caseload effectively.
• Excellent writing skills.
• Clear communication skills for providing outreach and consultation.
• Frequent travel throughout the Buffalo area required.
