Clinical Coordinator - Critical Care

This position coordinates the nursing care for assigned patients. Utilizing the nursing process, this role plans, implements, and evaluates nursing care in collaboration with the patient, family, and other health team members, demonstrating clinical competency and maintaining standards of professional nursing.

Responsibilities

• Collects, documents, and analyzes the patient’s current health status and medical history.
• Completes admission assessment.
• Formulates and initiates plan of care based on patient needs.
• Establishes priority of care based on physical, developmental, and psychological needs.
• Initiates nursing interventions based on relevant data collection.
• Evaluates and documents the effectiveness of interventions and modifies the plan of care and expected outcomes as appropriate.
• Applies nursing principles while monitoring patient condition and providing pain management and other interventions.
• Assesses patient/family learning needs, provides teaching materials, and documents teaching initiation, reinforcement, and completion.
• Administers medication using two unique patient identifiers and documents administration accurately, including effectiveness when appropriate.
• Scans patient and medication barcodes in Meditech 85% of the time on units that use BMV as applicable.
• Assists with or coordinates discharge planning programs.
• Implements, mentors, and adapts to new equipment, procedures, and technology that improves patient care.
• Assists in preparation for credentialing and licensing surveys (TJC, Medicare, etc.).
• Supports the hospital in meeting Core Measure goals, Press Ganey, Patient Satisfaction goals, and Performance Improvement Initiatives.
• Implements safety procedures for each patient in accordance with their safety needs.
• Maintains a clean and safe environment for all.
• Complies with safety standards: narcotic counts, incident reports, equipment failures, crash cart checks, and medication security.
• Collects and transports specimens in approved containers, labeling all specimens correctly in the presence of the patient when appropriate.
• Assists the Director/Manager with ensuring compliance with hospital nursing and unit policies, as well as professional, regulatory, and accrediting requirements.
• Participates in Performance Initiatives (PI) and Quality Blue Initiatives.
• Facilitates cooperative and collaborative relationships among staff and other patient care units.
• Coordinates delegated aspects of care and provides supervision and support to staff.
• Develops staffing plans considering personnel functions and appropriate staff mix.
• Functions as a resource person for staff.
• Participates in budget development, monitors daily operating budgets, reduces waste, and promotes patient care and safety.
• Ensures staff compliance with policies, procedures, standards of care, safety, and infection control functions.
• Identifies and addresses staff educational needs.
• Communicates equipment or supply needs to the Director/Manager.
• Actively participates in staff orientation.
• Monitors staff for ongoing needs and intervenes with behaviors not conducive to smooth operations.
• Attends and participates in staff meetings, taking an active role in problem-solving and staff remediation.
• Assists in staff evaluations, providing timely feedback on performance.
• Assists with interviewing, counseling, and performance competencies of staff.
• Participates in activities that meet identified learning needs and remains current with changes in healthcare delivery and standards.

Requirements

• Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC).
• Obtain certification in Basic Life Support within 30 days of hire date.

Qualifications

• Bachelor of Science in Nursing Degree (BSN) preferred.
• One (1) year of healthcare experience preferred.

• Proficient use of medical equipment.

Physical Requirements

• Able to bend, stoop, reach, and perform normal rotation, standing, and walking for an 11-12 hour shift.
• Able to ambulate, position, turn, and pull up self-assisting adult patients.
• Heavy/hard work: requires strength and stamina, including lifting, moving, stooping, reaching, standing, walking, and carrying materials and equipment weighing 40+ lbs.

Working Environment

• Working protracted or irregular hours.
• Must be flexible and able to take call.
• May be required to travel between facilities/buildings.
• May be exposed to viruses, diseases, infections, traumatic situations including psychiatric crises, physical injuries, and death.

Schedule

Scheduled Weekly Hours: 40
