Position Summary
The Clinical Coordinator assists the Nurse Manager in the directing, planning, evaluating, and coordinating of Member care on a nursing unit.
Essential Job Functions
- Assists in the completion of MDS.
- Assists in the review process to maintain accurate care cards.
- Understanding of and commitment to continuous quality improvement.
- Provides supervisory oversight on the nursing unit to ensure quality of care through: regular daily rounds; audit completion; care card review; monitoring compliance with turning and positioning; meal monitoring; coaching and teaching of staff on unit as issues occur; and providing family members with Member information.
- May be responsible for 1:1 staff teaching on the nursing unit.
- Assists with unit medical rounds.
- Aids in the auditing of quality of care and compliance with systems on a specific nursing unit.
- Aids the Nurse Manager in counseling and coaching.
- Provides input to the Nurse Manager for staff performance evaluations.
- Oversees staff compliance with mandatory in-services and schedules staff for programs to ensure attendance.
- May participate in the weekly interdisciplinary care plan meeting.
- Demonstrates excellent customer service skills.
- Ensures that all Members are receiving the necessary care and services to enhance and promote quality of life.
- Documents all required Member care information accurately and timely in My Unity.
- Responsible for unit management in the absence of the Nurse Manager.
- May be responsible for providing unit coverage on weekends or evenings and nights as necessary.
- Supports Departmental philosophy, goals, policies and procedures.
- Aids in the completion of Member documentation and monitoring for accuracy and timeliness.
- Notifies Member families with changes to Member plan of care.
- May be responsible for a unit medication pass when staffing levels are not optimal.
- Complies with HIPPA regulations.
- Performs other duties as assigned by the Nurse Manager.
- Ensures that Daily Team Conference is held on assigned nursing unit.
- Responsible for Member/Patient weight monitoring and documentation.
- Daily accountability to ensure all CNA documentation is complete.
Other Job Functions
- Dependable, report to work on-time for every schedule shift (ready to work at beginning - start of shift) and work full schedule shifts unless physically unable or pre-approved
- Paid Time Off (PTO) which includes personal and vacation
- Willing and able to work overtime and weekends as required by the position and/or approved by management
- Have reliable transportation to and from work
- Must be clean and neat in appearance and have a “friendly” service-oriented personality

Qualifications of the Position
- Education: Graduation from an approved LPN school of nursing.
- Experience: At least five years of progressive nursing experience.
- Skills: Ability to meet the public and participate in professional programs.
- Licenses / Certifications: Current New York State Registration and License.

Pay Range
$33.99 - $36.05 per hour
