Clinical Consultant Social Worker, Palliative Care (MSW/LSW)

About the Role

This position ensures organizational excellence related to customer service and quality of care while promoting excellence in the cultural experience at an assigned hospital. The role provides services to patients, families, and significant others, including biopsychosocial assessments, psychiatric diagnoses, treatment planning, therapy sessions (individual, couple, family, or group), treatment interventions, and discharge planning. The position supports and enhances biopsychosocial functioning and receives referrals for at-risk populations from interdisciplinary team members. The role requires the ability to act quickly and work in high-stress situations.

Responsibilities

• Clinical Assessment (30%)
  • Conducts patient/family/significant other interviews, chart reviews, and professional collaboration to assess biopsychosocial functioning.
  • Assesses developmental and age-specific needs, self-care abilities, dependent care, basic custodial and financial needs, social/emotional relationships, and compliance with treatment recommendations.
  • Develops care plans, including discharge plans, incorporating medical plans of care.
  • Identifies cases of abuse, neglect, or exploitation and refers them to legally mandated authorities.
  • Provides crisis intervention, information, referrals, and linkages to community resources.
• Intervention Management (30%)
  • Develops intervention plans based on assessments to resolve or support biopsychosocial functioning.
  • Employs clinical intervention skills, including treatment planning, continuum of care planning, supportive counseling, crisis intervention, and educational counseling through individual, family, or group modalities.
• Population Management (25%)
  • Coordinates clinical and financial outcomes for an assigned patient population.
  • Collaborates with interdisciplinary staff and external organizations for timely, cost-effective patient care management.
  • Monitors quality and type of services delivered to ensure management within established parameters.
  • Acts as a liaison to payers to manage post-discharge financial resources and ensure treatment progress.
  • Ensures completeness of treatment plans and timely reporting for assigned patients.
• Documentation and Regulatory Compliance (10%)
  • Thoroughly documents assessments and interventions in patient records and information systems according to department standards.
  • Complies with internal and external documentation requirements for patient/system needs and regulatory standards.
• Community Awareness and Referrals (5%)
  • Maintains awareness of current hospital and community services to support patient biopsychosocial functioning.
  • Provides information and facilitates linkages between patients and community resources.
• Performs additional functions as appropriate to the position.

Requirements

• Master’s Degree in Social Work (required).
• LSW (Licensed Social Worker) license in the State of Ohio.
• 1-2 years of experience as an LSW, preferably in a patient-centric setting.
• Knowledge of medical terminology and computer skills.
• Driver’s License (DL-HC) if working in a Home Care department.

Licensed Social Workers with a Bachelor of Social Work hired prior to 01/10/2019 are grandfathered into this role. Associates hired after 01/10/2019 must meet current qualifications.

Schedule

Day shift, 40 scheduled weekly hours.
