Clinical Case Manager RN Office Based

Key Responsibilities

• Provides oversight of all patient care services and personnel
• Covers making patient and personnel assignments, coordinating patient care, coordinating referrals, assessing patient needs, and developing, implementing, and updating the individualized patient plan of care
• Serves as the first line for resolution of concerns and issues for the staff
• Plays a key role in reviewing incoming referrals, triaging cases appropriately, helping determine eligibility and service needs, coordinating with intake and clinical staff, and ensuring smooth case flow from referral through admission readiness

Qualifications

• Active RN license in good standing
• Prior experience in home health, intake, referral management, case coordination, or clinical triage preferred
• Strong understanding of home health services, documentation, and care coordination processes
• Excellent communication, organizational, and multitasking skills
• Strong attention to detail and ability to manage multiple cases simultaneously
• Experience with Kinnser is a plus
• Knowledge of Medicare and other payer guidelines related to home health admissions
• Experience communicating with physicians, hospitals, rehab centers, and referral partners
• Ability to identify priority cases and escalate appropriately
• Comfortable using EMR systems and standard office software
