Clinical Case Manager, RN

How Will You Make An Impact & Requirements

• The Case Manager is responsible for the direct provision of services that assist patients with the achievement of their highest level of wellness.
• The Case Manager focuses on client-centered services that link clients and their family members with health care, and other home or community-based services to ensure timely, coordinated access to medically appropriate levels of health and support services and continuity of care.
• The Case Manager demonstrates knowledge of growth and development and possesses the ability to assess, interact, communicate, and care for age-specific patient populations.

Required Knowledge And Experience

• Registered Nurse; active and clear license in the State of Florida.
• The license must be maintained once employed.
• 3+ years of Clinical experience with 1+ years of case management experience preferred.

Required Skills And Abilities

• To perform this job successfully, an individual should have strong computer skills, especially in Excel, and knowledge of EMR systems; Athena knowledge preferred but not required.
