About the role

The Case Manager RN is a professional member of the healthcare team with 24/7 accountability for coordinating, monitoring, and managing patient care resources. This role ensures the efficient delivery of patient care services at the appropriate level of care while maintaining a high standard of care. The Case Manager RN acts as a resource for staff in defining, educating, implementing, and evaluating patient care and nursing practice standards using the nursing process, case management, utilization management, disease management, education, quality improvement, clinical practice, and healthcare trends.

Responsibilities

  • Assess complex patient care needs, situations, and acuity levels, gathering significant data from physical, functional, mental, psychosocial, environmental, family support, spiritual, cultural, financial, and legal assessments.
  • Coordinate patient care throughout an episodic or continuous course of care for specific DRGs, diagnoses, procedures, or populations using InterQual, Millikan, or other evidence-based criteria and guidelines.
  • Complete electronic documentation for case management activities, including discharge planning, patient throughput, appropriate level of care, and communication with physicians, nursing staff, and patients/family members.
  • Articulate information pertinent to clinical condition, length of stay (LOS), discharge delays, outcomes, resource utilization, and discharge planning at team meetings, presentations, patient care rounds, interdisciplinary rounds, and to administration, medical directors, and physician advisors.
  • Follow and perform all related unit-specific policies and procedures as detailed in the unit-specific competency checklist.
  • Utilize job and unit-specific equipment as required.
  • Demonstrate behaviors of service excellence and CARE values (Compassion, Accountability, Respect, and Expertise).
  • Perform all other related job duties as assigned.

Requirements

  • Bachelor’s degree in Nursing is required (employees hired prior to June 1, 2022, are grandfathered for the BSN degree).
  • Valid, unrestricted Florida RN license.
  • Case Management certification (CCM or ACM) is preferred.
  • Must meet and maintain all unit-specific and organizational skills, competencies, certifications, and licensures as required by regulatory and nursing standards of practice.
  • Generally requires 1 to 3 years of related experience.
