Case Manager, RN

Role Overview

This position involves making decisive judgments, building positive relationships, leading others, and taking appropriate actions to ensure effective patient care and safe transitions.

Responsibilities

• Provide education and resources on care progression, service utilization, appropriate care levels, and patient/family transitions.
• Conduct clinical assessments, apply critical thinking, make informed decisions, and organize care efficiently.
• Collaborate effectively with internal and external stakeholders, demonstrating strong interpersonal, written, and verbal communication skills.

Qualifications & Skills

• Minimum of three years RN experience, preferably in acute care or emergency settings, with knowledge of medical necessity criteria and discharge planning.
• Knowledge of Federal and State utilization review and appeals is a plus.
• A Bachelor of Science in Nursing is preferred.
• Certification of Case Management (CCM) is advantageous.
