Case Manager, RN

Role Overview: This position involves making decisive judgments, building positive relationships, leading others, and taking appropriate actions to ensure effective patient care and safe transitions. Responsibilities: Provide education and resources on care progression, service utilization, appropriate care levels, and patient/family transitions. Conduct clinical assessments, apply critical thinking, make informed decisions, and organize care efficiently. Collaborate effectively with internal and external stakeholders, demonstrating strong interpersonal, written, and verbal communication skills. Qualifications & Skills: Minimum of three years RN experience, preferably in acute care or emergency settings, with knowledge of medical necessity criteria and discharge planning. Knowledge of Federal and State utilization review and appeals is a plus. A Bachelor of Science in Nursing is preferred, and Certification of Case Management (CCM) is advantageous.
