Case Manager, RN

This role involves providing expert clinical assessment, critical thinking, and decision-making to support patient care and safe transitions. Responsibilities Include Leading and fostering positive relationships with healthcare teams, patients, and families.Offering education and resources to promote effective care progression and appropriate utilization of services.Ensuring compliance with utilization review and appeal requirements, with knowledge of federal and state regulations preferred. Qualifications Minimum of three years' RN experience, preferably with discharge planning in acute or emergency care settings.Solid clinical assessment, organizational, and interpersonal skills.Effective written and verbal communication abilities.Bachelor of Science in Nursing is preferred; CCM certification is a plus.
