About the Role

We seek a dedicated RN Clinical Care Manager to lead care coordination, clinical decision-making, and ensure high-quality patient care in a home health setting. This role involves managing the full patient journey, coordinating care across providers, handling physician orders and documentation within EMR, and conducting assessments to align care plans.

Responsibilities

• Lead care coordination and clinical decision-making for home health patients
• Manage the full patient journey from admission to discharge
• Coordinate care across multiple providers and disciplines
• Handle physician orders and maintain accurate documentation in EMR
• Conduct patient assessments and develop aligned care plans
• Lead case conferences and discharge planning
• Ensure regulatory compliance and adherence to care standards
• Support in-home visits as needed

Requirements

• Active RN license
• Home health experience preferred
• Strong clinical judgment and decision-making skills
• Excellent communication and collaboration abilities
• Experience managing complex cases and working independently
• Organizational skills and proactive issue resolution
• Accountability, compassion, and leadership qualities
