Clinical Care Coordinator

About the role

Under the direction of the Outpatient Director, implements, supports, and helps coordinate services for chronically mentally ill adults living in the community in conjunction with their individual treatment plans. This is a community-based position, meeting clients in the community rather than in an office setting.

Responsibilities

• Consumer Support: Provides Clinical Care Coordination, supportive counseling, and skill building for assigned clients. Supports and coordinates services, including assistance with income support and entitlements. Helps foster peer supports and facilitates community and family support. Ensures clients' mental and physical health needs are met. May provide Substance Use Disorder (SUD) services or work within related SUD grants. Demonstrates competency in screening, assessment, engagement, treatment planning, collaboration, referral, motivational interviewing, HIPAA, 42 CFR Part 2, and evidence-based counseling.
• Consumer Identification and Outreach: Provides outreach services to those in need within the community. Develops systems to deliver services to area clients. Ensures protection and advocacy of clients' rights. Provides referral information and assists with transportation as needed. Facilitates access to the intake process for non-clients.
• Information Management and Record Keeping: Completes clinical and administrative forms for billing, information, and clinical records. Ensures all case management documentation is completed within 24 hours and finalized within 3 business days. Attends staff meetings, agency meetings, in-services, and clinical trainings as required. Maintains confidentiality and follows program goals and agency policies.
• Communication: Ensures all individuals involved in a client’s care receive appropriate communication about care and planning. Performs other duties as assigned.

Requirements

• Minimum qualifications: High school diploma and training in Motivational Interviewing and Collaborative Network Approach modalities.
• Preferred: Bachelor’s degree in Social Work, Psychology, or related Human Services field, or equivalent experience of 4+ years.
• Valid Driver’s License.
• Computer Skills: Typing, navigating systems, and email usage.
