About the role
Berks Counseling Center is a recovery-oriented behavioral health, physical health, and supportive services organization providing a holistic approach to a culturally and economically diverse population for the purpose of achieving a healthy and productive lifestyle. As a Clinical Care coordinator, you will facilitate clinical care coordination services for clients in need of enhanced care coordination.
Activities include data collection and reporting, surveying, overseeing the coordination of care across programs, CQI activities, participating in huddles, and scheduling and assuring patient access to care. The coordinator will work closely with their direct supervisor and collaboratively with other department supervisors within the agency.
Responsibilities
- Provide outreach and a high level of customer service to referrals sources and collaborative partners.
- Provide direct administrative assistance to departments.
- Participate in overall compliance with all CCBCH, insurance, licensing and other standards and relevant laws/regulations that govern clinical care.
- Support internal operations within and across programs.
- Assist with the accuracy, timeliness, and compliance of information contained in the electronic health record (EHR).
Requirements
- Associate's degree plus at least one (1) year of experience working in a health or behavioral healthcare setting or in a similar capacity.
- Ability to work 40 hours per week within agency hours of operation, which are 8am-8pm Mon-Thurs, 8am-4pm Friday with Saturday hours, when applicable.

- COMMUNICATING: Must be able to communicate with patients, vendors, and coworkers.
- MOBILITY: Must have mobility to move about the agency.
- LIFTING/PUSHING/PULLING: Must be able to lift up to 5 pounds.
- OTHER: Must be able to write, type, and use office technology proficiently.
Qualifications
Preferred candidates will be Bilingual: English/Spanish.

Pre-Employment Requirements
