Location: The Valley Hospital, Paramus, NJ | Shift: Day

About the role

Coordinate and facilitate patient care through the Valley Hospital Health Care System using a multidisciplinary approach to patient progression, discharge planning, and utilization of resources.

Qualifications

• Master’s degree in social work from an accredited program
• New Jersey Social Work License required
• MSW and LSW or LCSW required; LCSW strongly preferred
• Minimum of three (3) years of experience as a Social Worker required

Skills

• Effective interaction and communication skills (oral, writing, presenting)
• Ability to perform work accurately with attention to detail while shifting between tasks without loss of efficiency or composure
• Ability to function competently in stressful situations and a changing work environment related to changing patient needs
• Ability to work cooperatively within the health system, with patients and family members, and with multidisciplinary team members (physicians, Rehab, Respiratory, DI, Lab, etc.)
• Effective time management to set priorities, perform job responsibilities, and respond quickly to emergency situations
• Critical thinking and clinical reasoning skills to effectively problem-solve and provide case management services

Pay

Starting salary and benefits in accordance with role, experience, education, and licensure. Actual individual salaries vary depending on these factors. The salary listed does not include other forms of compensation or benefits.

Pay Range: $41.30 – $51.62 per hour
