Duties & Responsibilities

Applied Population Health Analytics:
- Explore clinical, scheduling and external utilization data to create actionable information for front-line providers and senior leadership.
- Create and validate measures that illuminate how homeless patients are receiving care in New York City.
- Identify gaps in care that the program can address.
- Pull data from the Epic electronic medical record to track real-time patient needs.
- Harness and harmonize data across Epic and other data systems, and produce unified datasets that can answer important questions about how to deliver the best care to our most vulnerable patients.
- Design, maintain, and troubleshoot Tableau dashboards and reports to provide data-driven storytelling to our front-end providers and program colleagues.
- Explore clinical, scheduling and utilization data to create actionable information for providers and leadership, identify gaps in care, and produce unified datasets that can answer questions about how to deliver the best care.

Registry Work:
- Identify key reports to drive care and improve charge capture.
- Validate different generations of the registry which are produced.
- Bring registry data into a central SQL data warehouse.
- Develop and draft Tableau dashboards using registry data to provide program visibility to front line providers.
- Produce actionable patient lists to drive both regulatory and grant reporting and to improve individual patient outcomes.
- Provide analytics about churn, QA, and metrics for registries

Research and Evaluation:
- Act as the lead analyst for grant-funded health equity project on chronic kidney disease to assess the impact of the removal of a race adjustment for determining chronic kidney disease severity for Black/African-American patients.
- Analyze mammography and breast cancer data to create a care cascade for H+H patients to assess gaps in care and health disparities in breast cancer outcomes.
- Support the analysis of bias in clinical algorithms in H+H Epic EMR.
- Analyze utilization outcomes for the ED engagement and navigation program.
- Prepare abstracts and presentations on evaluation and research findings.
Minimum Qualifications
- A Masters Degree or advanced clinical degree from an accredited college or university and two years of experience serving as a lead in soliciting, defining, and managing requirements for internally and externally developed software in a clinical, allied health, non-allied health, behavioral health, information technology, information services, business, analytics or related area; or, one year of experience in a relevant clinical and/or healthcare administration role/function;
- A Baccalaureate Degree from an accredited college or university and five years of experience as described above;
- A satisfactory combination of education, training, and experience.
- Specialty certification(s) issued by a national commercially available program, state, professional society, academic or technical institution in an area(s) listed above, may be credited on a month-to-month basis toward the required work experience for a total of one year.
Department Preferences
- SQL and/or SAS/R are essential skills for this role.
- Tableau.
- Epic EMR system.
- Proven track record of management desired.
