Clinical Assessment Coordinator

Horizon Health is seeking a Clinical Assessment Coordinator to join our team at Abbeville General Hospital in Abbeville, LA. This role supports a 16-bed inpatient psychiatric unit treating patients ages 18 and older.

Responsibilities

  • Diagnose, implement, coordinate, and evaluate clinical admission, referral, and/or discharge processes.
  • Conduct on- and off-site patient assessments, including appropriate test administration, interpretation, and recommendations.
  • Respond to intake calls and complete admission paperwork.
  • Participate in the development of treatment plans in coordination with the treatment team.
  • Coordinate treatment with physicians and other health and social agencies; serve as an active member of the interdisciplinary team.
  • Utilize assessment tools including, but not limited to, Geriatric Depression Scale, Zung, LUMS, and alcohol screening tools under the authorization/supervision of an RN, LPC, or LMSW.
  • Communicate with referral sources including family members, the legal system, psychiatrists, primary care physicians, and other treatment facilities.
  • Review newly admitted patient accounts for eligibility and authorization daily.
  • Maintain schedule for filing court paperwork and hearings; work closely with court personnel to maintain good relationships.
  • Enter required data into the HorizonPlus system and generate monthly reports to track and trend referral, admission, and discharge data.
  • Perform other duties as assigned or required by this position.

Qualifications

  • Bachelor’s degree in nursing or master’s degree in social work/counseling required.
  • RN, LPC, or LMSW license required.
  • 3–5 years of intake experience required.
  • Crisis Prevention certification.
  • Enhance professional growth and development through participation in educational programs, in-service meetings, and workshops; stay current on professional literature.
  • Maintain effectiveness and certification in required patient management protocols.
  • Complete and maintain all required state/regulatory, hospital, and Horizon competencies and training/education expectations on a timely basis.
  • Knowledgeable of hospital and departmental policies and procedures, objectives, quality improvement program, safety, environmental, and infection control standards.
  • Develop basic knowledge of hospital and Horizon senior staff and organizational structure.
