About the Role

InnovAge is seeking a Clinic RN (Registered Nurse) to provide skilled nursing care to participants in accordance with InnovAge policies and all applicable regulations. This role involves assisting with examinations, testing, and treatment, as well as implementing care plans and ensuring high-quality healthcare delivery.

Responsibilities

  • Provide skilled nursing care to participants in compliance with InnovAge policies and regulations.
  • Assist in the examination, testing, and treatment of participants.
  • Implement orders written by primary care providers.
  • Triage participant and staff phone calls to determine priority of care and route accordingly.
  • Schedule outside appointments for participants when necessary.
  • Administer prescribed medications and treatments.
  • Train participants in self-administration of medications and monitor compliance.
  • Document all necessary information and maintain participant medical records.
  • Provide health counseling and education to participants and their families.
  • Observe, record, and report participant’s condition and reaction to drugs and treatments to providers.
  • Assist with clinical teaching and supervision of CNA Day Center staff.
  • Develop, implement, and maintain current nursing care plans.
  • Supervise LPN/LVNs and Medical Assistants, and evaluate clinic effectiveness in the absence of the Nursing Services Director/Clinic Nurse Supervisor.
  • Ensure exam rooms and triage areas are stocked with necessary supplies.
  • Communicate effectively and actively participate in interdisciplinary team meetings, family meetings, and clinic meetings.
  • Participate in the formulation of Plans of Care for participants within the interdisciplinary team.
  • Provide after-hours and holiday coverage as required.
  • Comply with all regulatory and policy guidelines.

Requirements

  • Current State-issued Registered Nurse License.
  • Current First Aid and BLS certifications (American Heart Association or American Red Cross).
  • 1+ year of experience working with the frail or elderly.

Preferred Qualifications

  • Bachelor’s degree in nursing.
  • 1+ year of acute care nursing experience.
  • 3+ years of healthcare experience with an emphasis in geriatrics.
  • Bilingual in Spanish, Russian, Punjabi, or Hmong.

Pay

Posted pay range: $41.73 - $54.28 per hour.
