Clinic Nurse (RN) - PRN

About the role

We are seeking a Registered Nurse to provide compassionate nursing care within a community-based healthcare program. The role involves developing and implementing personalized care plans, conducting initial and ongoing assessments, and coordinating care with interdisciplinary teams. The RN performs skilled nursing tasks, monitors participant conditions, and responds to urgent needs, including after-hours support through on-call rotations.

Responsibilities

• Develop and coordinate individualized care plans to promote safe, independent living.
• Conduct comprehensive assessments of new enrollees and periodic re-evaluations.
• Administer general and skilled nursing care, follow physician orders, and document participant conditions.
• Triaging and providing urgent care, including participation in after-hours on-call duties.
• Collaborate with care teams to ensure seamless service delivery.

Qualifications

• Graduate of a professional nursing program.
• Current Colorado RN license and CPR certification; BSN preferred.
• Valid driver’s license, proof of insurance, and reliable transportation.
