About the role
Piedmont Health Senior Care offers the Program of All-inclusive Care for the Elderly (PACE) to enhance the lives of seniors in the community. We aim to help seniors maintain their independence and live at home for as long as possible by providing comprehensive, personalized healthcare and related services tailored to each senior's unique needs and aspirations.
Responsibilities
- Utilizes the nursing process including: assessment, planning, implementation, evaluation, and recording of patient care and patient education.
- Provides support to providers by anticipating needs and providing assistance in the provision of care.
- Captures services with other members of the health-care team, internal and external, to provide complex patient care.
- Answers or returns phone calls.
- Provides onsite and telephone triage.
- Serves as a member of the interdisciplinary care team and participates in huddles.
- Performs other duties as assigned.
Qualifications
- Education: Practical Nursing Accredited Program, License Practical Nurse Diploma or Certificate.
- Licensure, Registry or Certification Required: Current & Valid NC License Practical Nurse; Basic Cardiac Life Support (BCLS).
- Experience: One year of general Licensed Practical Nurse experience preferably within the last three years. Preferred experience: Previous experience in an ambulatory health care setting; bi-lingual.
- Immunizations: Be medically cleared for communicable diseases and have all immunizations up-to-date prior to beginning employment.
