About the role

CommuniCare+OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs.

Responsibilities

  • Facilitate communication among providers, staff, and patients.
  • Manage and work with various schedules to maximize clinic flow.
  • Oversee and direct the stand-by list to ensure effective communication between front and back office.
  • Work collaboratively with the Clinic Manager, Medical Director, Access Supervisor, Lead Medical Assistant, and Back Office Supervisor to fill appointment slots.
  • Screens patients for clinic visits and coordinates case information.
  • Supports nursing staff to minimize wait times for patients, especially those with urgent needs.
  • Works collaboratively with medical assistants to support quick starts and QIP measures.
  • Maintains patient confidentiality in accordance with CommuniCare+OLE’s policy and HIPAA guidelines.
  • Assists with medical visits, patient interviews, and other aspects of patient care.
  • May assist with calling patients to schedule appointments.
  • Supports cleaning and stocking of patient care areas and workstations.
  • Attends monthly update meetings, in-services, and trainings.
  • Assists with inventory, ordering, and stocking of medical supplies.
  • Supports remote healthcare providers by coordinating patient referrals and communicating test results.
  • Handles backline calls and supports newborn registration and scheduling.

Qualifications

  • Completion of M.A. certification required.
  • Phlebotomy certification preferred.
  • Two years working as an M.A. in an outpatient setting.
  • Demonstrated commitment to low-income populations.
  • Bilingual in English/Spanish/Dari/Punjabi/Vietnamese strongly preferred.
  • Knowledge of Federally Qualified Health Centers and key health center principles and guidelines.
  • Experience with computer systems, electronic health record system, spreadsheets, and scheduling guidelines.
