Job Summary

The Client Support Coordinator provides direct services to individuals enrolled at Profiles Day Program under the direct supervision of the Day Services Director. The role involves providing services including observation, supervision, training, assistance, and support in areas of need outlined in the IPP. The position is a full-time (40 hours/week) daytime position.

Job Duties and Responsibilities

  • Improves client participation in the ADDT program by identifying and reducing barriers to client engagement through observation, intervention, and training.
  • Maintains strict confidentiality for all consumer information and facility operations.
  • Serves all consumers of the program and interacts with both direct care and admin staff in a positive and professional manner.
  • Participates in meetings with clients, caregivers, direct care staff, administrative staff, and outside vendors as requested by the Day Service Director.
  • Serves as the first point of contact for Profiles staff when additional support is needed for consumers exhibiting disruptive behaviors.
  • Acts as a liaison between consumers and staff when identifying breakdowns in communication.
  • Affords assistance to consumers in accessing solutions to their concerns to facilitate appropriate participation in the day program.
  • Completes reporting requirements per ILS and licensure policy for incidents as indicated.
  • Observes consumers to identify barriers to participation and works to develop effective solutions and trains other team members as indicated.
  • Plans and hosts monthly training sessions, workshops, meetings, etc. with clients and/or staff to maximize outcomes for the consumers participating in the ADDT program.
  • Makes recommendations to the care team regarding consumer classroom placement and schedule.
  • Maintains an updated electronic summary database that includes consumer roster with precautions and behavioral notes. Keeps updated information available at all times to Profiles staff.
  • Affords assistance with ADDT staff to consumer ration if needed.
  • Affords assistance with client personal care as needed.
  • Affords assistance with setup and cleanup for large-group activities.
  • Attends relevant trainings and conferences and presents information to team upon return.

Qualifications

  • Two years of experience working with individuals with developmental disabilities.
  • A bachelor's degree in a related field may be considered in lieu of experience.
  • Initiative, self-direction, and the ability to perform assigned tasks without direct oversight.

Hiring Qualifications

  • High School Diploma or equivalent.
  • Valid Arkansas driver's license.
  • Ability to safely operate any and all vehicles, business machines, and industrial equipment necessary for the successful operation of the program.
  • Proof of current vehicle liability insurance.
  • Ability to lift 50 pounds.
  • Maintain current CPR and First Aid Certification.
  • Successfully complete a minimum of 12 hours continuing education and training in topics relative to the position annually.
  • Experience in areas of employment directly related to the performance of this job as included in the above "Job Duties and Responsibilities" is preferred.
