Client Stabilization Specialist
Job Summary
The Community Bridges, Inc. (CBI) Client Stabilization Specialist is a shelter or temporary lodging site-based position. The primary role involves working with individuals and families experiencing homelessness, ensuring they are stabilized, conducting intakes into HMIS, providing education, resources, and overall orientation, conducting face-to-face visits, perimeter walks, and escalating issues impacting stabilization.
The position communicates with leadership and Client Care Coordinators on program violations to address issues and ensures proper coordination of services. It also identifies the need for a housing prioritization assessment for all individuals.
Skills/Requirements
- Highschool diploma or GED
- Minimum of six months of recovery from substance use and/or mental health disorders
- 1-3 years of full-time health care related work experience
- Valid identification required
- Behavioral Health Technician (BHT) certification in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures preferred
- CBI Peer Certification within 90 days of hiring
- AZ Fingerprint Clearance Card (program specific)
