About the role

Help Bring Exceptional Pediatric Home Care to More Families
As Affirmed Home Care continues to expand our Pediatric Division throughout Connecticut, we are seeking an experienced, results-driven Client Liaison to develop strategic referral partnerships, grow our referral network, and increase access to exceptional pediatric home care services.

Responsibilities

• Build and maintain referral relationships with hospitals, physician practices, case managers, discharge planners, school districts, and community organizations throughout Connecticut.
• Generate new pediatric home care referrals by leveraging existing relationships and developing new partnerships.
• Represent Affirmed Home Care at networking events and community outreach activities.
• Educate referral partners on our pediatric home care services.
• Collaborate with Clinical, Intake, Recruiting, and Marketing teams to ensure an exceptional referral experience.
• Track referral activity and achieve business development goals.

Qualifications

• Bachelor's degree in Business, Marketing, Healthcare Administration, Communications, or a related field preferred.
• Established referral network and book of business throughout Connecticut in pediatrics strongly preferred.
• Strong understanding of pediatric specialty care, home healthcare services, and referral development.
• Strong communication, networking, and relationship-building skills.
• Valid driver's license and reliable transportation.

Pay

Starting at $120,000+ Base Salary + Commission + Potential Signing Bonus
