What You'll Be Responsible For
- Work closely with the Client Care Coordinator and Staffing Coordinators to ensure seniors receive the highest quality of care and service.
- Work directly with clients and families, assess changing needs, support caregivers, and help ensure clients can continue receiving the care they need in the comfort of home.
- Communicate with clients, families, and caregivers to answer questions and address client-care concerns.
- Perform client assessments and develop and maintain individualized plans of care.
- Assess clients following a change in condition, hospitalization, or change in level of care.
- Maintain accurate and up-to-date client records and care plans.
- Complete required paperwork and documentation according to state guidelines.
- Conduct Quality Assurance visits with high-level companion/home helper clients.
- Complete Supervisory Visits as needed.
- Review, maintain, file, and audit personal care notes.
- Teach Personal Care classes.
- Maintain open communication with the owner, office colleagues, clients, and caregivers.
- Handle designated weekend on-call responsibilities, including client-care calls, emergencies, care consultations, and caregiver questions.
