Responsibilities

  • Conduct start-of-care visits, supervisory visits, and client assessments to ensure quality care delivery.
  • Develop, review, and revise individualized plans of care based on patient needs.
  • Supervise homemakers, personal care aides, and home health aides to ensure compliance and quality standards are met.
  • Maintain accurate clinical documentation, care plans, and service records.
  • Communicate professionally with caregivers, clients, case managers, and leadership teams.
  • Participate in staff meetings, in-services, and continuing education opportunities to support professional growth.
  • Evaluate home environments for safety risks and recommend solutions to improve patient safety.
  • Support Elara Caring’s mission, values, and commitment to exceptional patient-centered care.

Requirements

  • Graduate of an accredited School of Nursing.
  • Current Registered Nurse (RN) license in good standing in the state applying for.
  • CPR Certification required.
  • Minimum of one (1) year of nursing experience; Home Care experience preferred.
  • Knowledge of Medicare/Medicaid, home care benefits, and applicable State regulations preferred.
  • Strong clinical assessment, communication, and organizational skills.
  • Excellent computer proficiency and documentation abilities.
  • Ability to multitask and adapt in a fast-paced healthcare environment.
  • Professional verbal and written communication skills.
  • Ability to motivate and lead caregivers in a positive and supportive manner.
  • Reliable personal transportation is required.
  • Ability to travel locally, and regionally to support additional field offices as needed.
