About the role

The Client Care Coordinator is responsible for the overall delivery and coordination of mental health and related services for homeless individuals. This role enhances existing services by conducting thorough assessments of clients’ long-term goals at intake and collaborating with case management staff to set achievable goals. The Coordinator establishes community linkages to support clients’ progress toward independent living and works with clients at the Poplar location in the Bronx, NY.

Responsibilities

  • Complete a comprehensive bio-psychosocial assessment with each client to understand strengths and service needs.
  • Provide clear written reports capturing assessment findings and recommendations.
  • Assist homeless men in navigating multiple systems and coping with stressors and anxiety induced by homelessness.
  • Provide outreach services, crisis intervention, risk assessment, safety planning, and psych-education.
  • Prioritize and conduct unit visits for high-risk clients in-shelter to coordinate safety planning efforts.
  • Prepare clients to accept services and collaborate with ACS and/or prevention services agencies to advocate on their behalf.
  • Make appropriate referrals and facilitate linkages between mental health providers, government agencies, and community-based services.
  • Serve as an agency/program liaison between government agencies, community-based organizations, and groups.
  • Improve access to mental health services by facilitating in-person or telehealth services.
  • Work effectively as part of a multi-disciplinary team to enhance engagement and ensure quality service.
  • Deliver enhanced mental health services, including biopsychosocial assessments, on-site mental health services, clinical care coordination, referrals, and follow-up.
  • Promote telehealth usage through H+H Express Care and other providers of the client’s choice.
  • Utilize DHS Referral Basics to match client needs, assist with scheduling, coordinate transportation, and provide warm hand-offs.
  • Follow up on referrals to confirm attendance, address concerns, and assist with rescheduling if needed.
  • Utilize motivational interviewing to re-engage clients who are hesitant about treatment.
  • Provide therapeutic groups (e.g., music, art, support groups) to address family issues and enhance life skills.
  • Participate in family meetings with case management and housing staff to discuss planning efforts and progress.
  • Assist clients with completing applications for supportive housing and accompany them to interviews.
  • Upon linkage to permanent housing, complete and discuss an exit summary with the client and provide referrals for supportive services in the new community.
  • Utilize fact sheets, desk guides, and other resources to deliver mental health services.
  • Complete all documentation in a timely manner, including services provided, efforts, and outcomes in CARES.
  • Compile information and data for internal and external reports.
  • Attend professional development training and FWC mental health trainings delivered by DHS.
  • Perform other duties as assigned.

Requirements

  • Must be a Licensed Master Social Worker (LMSW) in the state of New York upon hire or obtain licensure within three months of hire.
  • Master’s degree in social work from an accredited school of social work.
  • At least one year of experience in social services, counseling, crisis intervention, family services, preventive services, housing services, or homeless services (strongly preferred).
  • Knowledge and experience working with diverse cultures and ethnicities.
  • Expertise in strengths-based, solution-focused, and family-centered practice.
  • Experience facilitating groups for adults.
  • Proficiency in Spanish and/or French is a plus.
  • Excellent written and verbal communication skills.
  • Excellent interpersonal skills and computer literacy in Microsoft Office.

  • Ability to lift and move objects weighing up to 30 pounds several times a week.
