Responsibilities
- Conduct in-home quality assurance visits and client assessments
- Cookordinate care services and maintain strong client and family relationships
- Introduce clients and Care Professionals to ensure successful care transitions
- Build referral relationships with healthcare providers and community organizations
- Represent Home Instead at community events, health fairs, and networking activities
- Provide direct client care coverage as needed
- Resolve client concerns with professionalism and compassion
- Maintain accurate documentation and ensure compliance with company policies
- Collaborate with office staff, schedulers, and healthcare professionals to support quality care
Qualifications
- College degree or equivalent work experience preferred
- CNA license preferred
- Valid driver's license and reliable transportation required
- Excellent communication and interpersonal skills
- Strong organizational, problem-solving, and time management abilities
- Ability to work independently while maintaining confidentiality
- Proficiency with Microsoft Word, Excel, and technology systems
- Willingness to work occasional evenings, weekends, on-call shifts, or direct care assignments as needed
