Clerical Assistant – Inpatient Care Management | Part Time

As a Clerical Assistant in Inpatient Care Management, you’ll support the team in facilitating safe, timely transitions for hospitalized patients. From scheduling follow-up appointments to arranging transportation and delivering regulatory notices, you’ll play a key role in ensuring continuity of care and compliance with quality standards. Responsibilities
- Perform general office and secretarial duties to support care coordination and discharge planning
- Deliver the Important Message to Medicare beneficiaries and assist with the appeal process when needed
- Schedule post-discharge follow-up appointments to meet quality performance metrics
- Meet with patients to educate them about appointments and identify barriers to attendance
- Arrange patient transportation as indicated
- Assist in the development and implementation of departmental projects and performance improvement initiatives (Katas)
- Maintain accurate documentation and support efficient department operations

Qualifications
A high school diploma or equivalent is required, with an Associate degree preferred. Previous clerical experience in a healthcare environment is also preferred, as is experience with Microsoft Office, fax machines, and electronic medical records.
