Job Summary
The Chronic Care Specialist supports patients with high-risk health conditions in navigating the healthcare system. They collaborate with multidisciplinary care teams to manage chronic care and transition of care processes. The role involves acting as a resource for clinic staff and performing nursing duties within the scope of an RN/LVN/LPN license.

- Experience related to patient education and/or motivational interviewing skills and self management goal setting.
- Experience with outbound patient telecommunications.
- Bilingual: English and Spanish preferred.
- Excellent verbal and written skills.
- Solid interpersonal skills.
- Ability to interact productively with individuals and with multidisciplinary teams.
- Excellent organizational and prioritization skills.
- Ability to travel throughout the assigned region and conduct home visits as needed.
- Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF, HTN, and CRF.
- Proficient computer skills to work efficiently with electronic medical records.
Job Requirements
- Licensed Practical/Vocational Nurse or other credentialed health professional as approved.
- With an active and unrestricted NM or Compact State License.
- Current BLS certification.
- 2+ years of experience in a physician's office, clinical or hospital setting.
