CHMC-RN Case Manager

About the role

We are seeking a highly organized and clinical RN Case Manager to oversee the care of complex patients. In this role, you will act as a central coordinator, negotiating and managing resources to ensure high-quality, cost-effective outcomes from admission through discharge.

Responsibilities

• Manage complex patient cases by collaborating with interdisciplinary teams both inside and outside the organization.
• Merge clinical needs with financial processes, interpreting Medicare, Medicaid, and private insurance benefits to facilitate care.
• Participate in performance evaluation processes to enhance patient care across the entire continuum.
• Maintain accurate records, ensure patient confidentiality (HIPAA), and adhere to all regulatory and safety standards.
• Use evidence-based best practices and disease management knowledge to guide treatment regimens and patient safety.

Qualifications & Requirements

• Current New York State RN License and registration.
• Minimum of two (2) years of nursing experience.
• Current BLS certification.
• Basic computer proficiency and the ability to navigate complex medical/financial data.
• Ability to stand, bend, and walk for extended periods.

Pay Range: $37.63–$49.86 per hour
