Role Overview: This position involves assessing, developing, implementing, and evaluating patients' discharge planning needs in collaboration with an interdisciplinary team. It ensures compliance with discharge planning standards, identifies and addresses gaps related to inpatient and observation stays, and assesses high-risk patients for readmission, developing appropriate care plans. The role may also include managing utilization, emergency department case management, and acting as a liaison with post-acute providers. Responsibilities Ensure compliance with discharge planning regulations and standards.Identify barriers and escalate issues related to patient stays.Assess high-risk patients, analyze root causes of readmission, and collaborate on discharge plans.Participate in quality improvement initiatives and standard work evaluations.Perform utilization management, ED case management, and act as liaison with post-acute providers.Apply evidence-based practices, adhere to professional nursing standards, and follow the ANA Code of Ethics.Maintain safety awareness and perform additional duties as assigned. Qualifications & Skills Graduate of an accredited nursing program or equivalent.Minimum three years of acute care experience.Current RN license in applicable states with required certifications.Physical ability to perform lifting, standing, walking, and other movements.Excellent communication, assessment, and collaboration skills.
