In coordination with therapeutic staff, the Case Manager will assist youth and families by providing information, education, and training to build knowledge, develop skills, and change attitudes with a goal of increasing the consumer’s independence, productivity, self-determination, integration, and inclusion in the community. The majority of time will be spent providing services to youth in the family home, educational setting, program site or other community settings, with minimal time spent at the Center. This position reports directly to the CBS Coordinator or designee. Major Job Responsibilities Participates in treatment planning based on best practice theories and applies interventionsDevelopment of personal plan/goal sheet in conjunction with consumer's needsObtains location and the process of acquiring resources; coordinates with area agenciesDirect involvement in planning and advocacy in areas of: Education/Vocation, Residential, Financial, Health , Leisure, and Social SupportsEducation to family in symptomatology, awareness of relapse indicators and handling of crisisAssists in collection of program data and provides reports as necessary to immediate supervisorWorks in conjunction with other mental health center staffMust provide transportation to and from various program activities ?(must maintain a good driving record)Meets direct service expectations of 47% of time workedWorks evening hours each week unless direct service and client needs are metMeets deadlines for daily paperwork and reportsProvides quality documentationProvides supportive intervention when needs arise outside of scheduled hoursAssists other case managers and consumers as needed to help meet agency’s needsPerforms other duties as deemed appropriate by immediate supervisor or designeeCompletes all trainings as assignedComplies with ELC policies and procedures ELC is an Equal Opportunity Employer (EOE) and considers all applications irrespective of race, marital status, sex, age, skin color, religion, nationality, veteran status, disability or any other characteristic protected by law. Requirements REQUIRED QUALIFICATIONS: Bachelor’s degree or equivalent experience necessary to meet State of Kansas licensing regulationsPossesses the minimum qualifications as documented in the Kansas Case Management Program StandardsFamiliarity with mental health theory and practiceClean driving recordIncumbent must pass a security check as required by Medicaid Regulations Knowledge, Abilities, And Skills Relates a positive approach to complex mental health issuesAbility to communicate effectively orally and in writingAbility to establish effective working relationships with staff, community agencies, and consumersAbility to be sensitive to cultural and ethnic differencesAbility to safely perform major job responsibilities, with or without reasonable accommodations
