Children Case Manager
About the role
Southwest Behavioral and Health Services is seeking a dedicated and empathetic Case Manager to join our dynamic Bullhead City team. The Case Manager serves as a central support within the interdisciplinary team, working primarily with children. This staff member conducts assessments, assists members with resources within the community and provides coordination of care inside and outside of the agency.
Responsibilities
- Completing intake paperwork and referrals
- Completing annual behavioral health assessments, including service planning and support planning
- Facilitating CFTs and ARTs
- Facilitating groups
- Identifying gaps in care for our members
- Completing SMI and SED evaluations
Qualifications
- Minimum of 22 years of age
- Bachelor’s degree in the behavioral health field plus one year of relevant experience in behavioral health
- A combination of experience and education totaling 2 years is acceptable
- Valid AZ driver’s license & motor vehicle clearance
- Must have fingerprint clearance issued by the Arizona Department of Public Safety or be eligible to receive upon job offer
