Started in 2000 by a group of committed volunteers, Charlotte Community Health Clinic offers high-quality medical, dental, and behavioral health services for kids and adults. The clinic is launching Annual Wellness Visits and Chronic Care Management for their Medicare population.

About the role

This fellowship focuses on developing and implementing pharmacist-led population health services, including:

• Annual wellness visits (AWVs) for Medicare patients
• Annual comprehensive visits for Medicare patients
• Chronic care management (CCM) for Medicare patients with 2 or more chronic conditions
• Remote physiologic monitoring (RPM) for digitally collected physiologic data
