Charge Nurse - Full-Time

About the role

Charge Registered Nurse (RN) responsible for coordinating and delivering clinical care within a PACE center, supporting elderly participants to live independently in the community.

Responsibilities

• Oversee daily clinical activities and workflows.
• Provide clinical leadership and support to the care team.
• Manage participants' health conditions in collaboration with providers.
• Administer medications, treatments, immunizations, and wound care.
• Conduct assessments and contribute to care planning.
• Assist with staff onboarding, training, and inventory management.
• Support quality and compliance initiatives, participate in on-call rotations.

Qualifications & Skills

• Current RN license, graduate of an accredited nursing program.
• Preferred leadership experience as a Charge Nurse.
• Experience with elderly or long-term care populations.
• CPR certification or ability to obtain.
• Strong clinical judgment, communication, and organizational skills.
• Participant-focused and compassionate approach.

• Meet CMS competency standards.
• Flexibility to work beyond normal hours and in varied environments.
