Charge Nurse - Full-Time

Role: The Charge Registered Nurse (RN) supports daily clinical care coordination for elderly participants in a community-based program offering comprehensive services through an interdisciplinary team. Responsibilities Include Overseeing clinical care workflows and providing leadership to the care team.Monitoring, assessing, and managing participants' health conditions in collaboration with providers.Administering medications, treatments, immunizations, and wound care.Conducting nursing assessments and contributing to care plans.Supporting staff onboarding, training, and quality initiatives.Managing inventory, equipment, and ensuring compliance with regulations.Participating in on-call and holiday rotations as needed. Qualifications And Skills Current RN license and graduate of an accredited nursing program.Preferred: two years of leadership or Charge Nurse experience.At least one year working with elderly or long-term care populations.CPR certification or ability to obtain.Strong clinical judgment, communication, organizational, and leadership skills.Compassionate, participant-focused approach. Additional Requirements Willingness to work beyond normal hours and in varied environments.Must meet CMS competency standards prior to independent work.Fully vaccinated or willing to be vaccinated against specified illnesses.
