Responsibilities

• Serves as a leader of the healthcare team at Novant Health
• Establishes an authentic personalized relationship with the patient and their chosen support system
• Collaborates with physicians and other team members to assess, plan, implement, and evaluate an individualized plan of care
• Assists the patient in navigating their health journey
• Manages and provides care in accordance with current policies and procedures

Requirements

• 4 Year / Bachelors Degree, preferred
• One year relevant experience, preferred
• Current RN licensure in appropriate state, required

Qualifications

• Committed to compassionate care and teamwork
• Willing to relocate to Bluffton, SC
• Willing to work weekends and holidays
