Job Summary

The Chaplain / Volunteer Coordinator at Caris Healthcare supports the hospice interdisciplinary team through office functions. Key responsibilities include completing comprehensive patient assessments, providing spiritual support, conducting visits, and connecting patients and caregivers with community resources.

Responsibilities

• Completes and documents ongoing spiritual assessments, interventions, and outcomes related to patient and family
• Communicates significant spiritual needs of the patient and family to the hospice interdisciplinary team
• Conducts visits with patients and caregivers in their residential setting
• Connects patients and caregivers with community clergy and/or spiritual leaders as needed
• Assists with bereavement support and interventions for caregivers following the death of a patient
• Participates in an on-call rotation for after-hours visits

Qualifications

• Master Degree of Divinity, Religion or Theology
• One unit of Clinical Pastoral Education (CPE) is preferred, for Georgia 1 CPE is required
• One (1) year experience in Hospice or related field
• Excellent communication and interpersonal skills
