About the role
VIA Health Partners is seeking a Spiritual Care Provider to join our dynamic team. This role involves providing spiritual care to patients and caregivers, coordinating with interdisciplinary teams, and facilitating anticipatory grief services.
Responsibilities
- Provides spiritual care including prayer, sacraments, religious rites, and education.
- Develops and implements a plan of care incorporating spiritual care interventions.
- Maintains a liaison relationship with community spiritual care resources.
- Communicates with interdisciplinary team members and updates the plan of care as needed.
- Participates in interdisciplinary team conferences and special case conferences.
- Provides anticipatory grief services and referrals to grief care services.
- Schedules and prepares for visits, ensuring continuity of care.
- Documents clinical information accurately and completes required reports.
- Maintains professional relationships with interdisciplinary teams and staff.
- May provide memorial services and assist with bereavement support.
Requirements
- Master’s Degree in Divinity from an accredited seminary with BCPE units.
- At least two years of related experience in a healthcare setting.
- Ordination or endorsement by a recognized religious body.
- Two years of clinical experience in death and dying, preferably in CPE.
- Three years of experience in a community of faith or health care chaplaincy.
- Board certification/eligibility preferred.
- Basic computer skills, including Microsoft Office.
Qualifications
