About the role
Continuum Rehab Group offers in-house therapy within Skilled Nursing facilities in Texas, New Mexico, Arizona, Indiana, and Missouri. Responsibilities
- Utilizes evidence-based practice to support clinical interventions.
- Provides rehabilitative, skilled, and medically necessary treatment intervention to patients with physical functioning disorders under the supervision of an Occupational Therapist.
- Provides skilled therapy services to patients requiring rehabilitation services.
- Affirms and documents patient progress and communicates with supervising Occupational Therapist to adjust treatments as needed.
- Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
- Contributes to the discharge summary of services provided according to regulatory and clinical requirements.
Qualifications
This position is open to therapists at all experience levels. Required qualifications include:
- Licensed Occupational Therapy Assistant.
